Adams Central Athletics

A new IHSAA sports physical is required for the 2025 – 26 school year. The current (2024-25) physical will expire on May 31, 2025. If your child (grades 6-12) plans to participate in any summer activities such as conditioning, open gyms, camps, etc., they must have Final Forms completed along with a new sports physical on file in order to attend. All student-athletes have three requirements to be able to participate in athletics at Adams Central. 1. Final Forms Log-in a. Only needs to be done once per household b. How to Register – SEE ATTACHED 2. Final Forms Documents a. First year form completion with yearly review / update as needed b. Final Forms Documents – SEE ATTACHED c. Print the “IHSAA PPE Physical Form” to take to your medical provider d. How to Switch to New Year (2025 – 2026) – SEE ATTACHED 3. Pre-Participation Physical dated on or after April 1, 2025 a. New physical required for each school year b. Bring completed page 3 to Athletic Office or upload to Final Forms account. IMPORTANT – Athletes will not be allowed to participate until both the physical is on file and Final Forms documents have been completed by both the parent & student.
